Enrollment Procedures at CHS
If you need to enroll your student please call Merly RuizAke, Counseling Secretary at 503-844-1814 to confirm that Century is your home school. She will make an appointment for student and parent with the appropriate grade level counselor to complete your enrollment. Please allow 1 hour for enrollment appointments. During this time, your counselor will help with the transition to Century, talk about your class selections/forecasting and goals for high school.

When enrolling your student you will need the following:
- Proof of address from the past 30 days (utility bill, copy of purchase/rental agreement)
- Immunization Records
- Proof of legal name (birth certificate, current passport, driver's license or permit, state ID card)
- Transcript from previous school if enrolling in 10th-12th grade to help determine class placement
- IEP or 504 paperwork if appropriate
- Completed Enrollment forms English Spanish
- Completed Home Language Survey English Spanish Vietnamese
- Completed Google Apps Parent Permission Form English Spanish
